The patient recruitment form provides the user with the ability to filter patients based on specific criteria or series of criteria:

1.    Demographic Information (i.e., Sex, Age, Race, State, Zip, etc.) (See Recruitment Demographic Pop-up.)

2.    Lifestyle Information (See Recruitment Lifestyles Pop-up.)

3.    Medical History (See Recruitment Medical History Pop-up.)

4.    Medications and Medication Classes (i.e., Beta Blockers) (See Recruitment Medications Pop-up.)

5.    Allergies (See Recruitment Allergies Pop-up.)

6.    Study History and Recruiting Source (See Recruitment Study History Pop-up.)

 

A detailed patient report or a summary report may be printed for the list of recruit-able patients. Also, the patient’s address and first name may be sent as a text file for merging into a word processor to create a Mail Merge.
