The Provider Visits report includes
1. List of procedures performed by provider
2. Patient and date information for procedures
3. Status for procedures
Information can be printed for all providers for a study, a specific provider, a specific procedure, and starting and ending dates. A parameter can be left null to omit it as a selection.
