The Patient Visit Notepad includes
1. Patient information
2. Procedure information
3. Visit notes
4. Space for procedure notes
5. Lines for provider signatures
Information is based on the user-selected date, patient, and visit number from a preceding parameter form. If a patient is not selected, ALL scheduled visits will be printed for a given date.
HINT: This form may be used to collect source document information.
